Register for your first dose of Moderna at the State of Delaware’s vaccination event on Saturday, April 24 from 9 a.m. to 5 p.m. at Dover International Speedway.

WHO?

  • All Delaware residents 18+
  • Individuals 18+ who work, attend college, or receive health care in Delaware

WHEN?

  • Saturday, April 24, 2021. Appointments are available 9:00 a.m. – 5:00 p.m.

WHERE?

Dover International Speedway

1000 Leipsic Road, Dover, DE 19901 (use this entrance)

This is a drive-through event.

HOW?

FILL OUT THIS FORM: Please print and fill out the vaccination information form and pre-vaccination screening found at here. ONLY FILL IN THE FIELDS MARKED IN YELLOW (do not fill in the fields marked in green). The pre-vaccination screening questions are about your health on the day of your appointment. If you cannot print and fill out the form in advance, copies will be available on site. This form is required for a second dose even though you filled it out for your first dose.
